SENATE AMENDMENT NO. 1

TO

SENATE BILL NO. 76

AMEND Senate Bill No. 76 by inserting after line 13 the following:

“(2) Satisfactory proof that the applicant is a veteran includes any of the following:

f. PHS Form 1867, Statement of Service – Verification of Status of Commissioned Officers of the U.S. Public Health Service; also referred by the short title, Statement of Service.

g. NOAA Form 56-16, Report of Transfer or Discharge.

”.

SYNOPSIS

This Amendment adds 2 forms to the list of documents that qualify as proof that an applicant is a veteran, to ensure that a member of the United States Public Health Service Commissioned Corps or the National Oceanic and Atmospheric Administration can receive veteran designation on the applicant's driver license.
