Dear Chairman Mendelson:
Pursuant to section 451 of the District of Columbia Home Rule Act (D.C. Official Code § 1 -
204.51) and section 202 the Procurement Practices Reform Act of 2010 (D.C. Official Code § 2 -
352.02), enclosed for consideration and approval by the Council of the Di strict of Columbia is
proposed Modification No. M00005 to Human Care Agreement No. CW101973 with Wholistic
Home and Community Based Services, Inc. to exercise option year two in the not -to-exceed
amount of $1,282,844.50. The period of performance is from March 1, 2025, through February 28,
2026.
Under the proposed modification, Wholistic Home and Community Based Services, Inc. will
continue to provide occupancy-related residential expenses and services not covered by Medicaid
to individuals with intellectual and developmental disabilities. These services are mandated by the
Citizens with Intellectual Disabilities Constitutional Rights and Dignity Act of 1978, effective
March 3, 1979 (D.C. Law 2-137, D.C. Official Code § 7-1301.02 et seq.).

recovers any outstanding debt as provided under D.C. Official Code § 2-353.01(b):
The Citywide Clean Hands database certified contractor as being in compliance with the Office of
Tax and Revenue and the Department of Employment Services taxes on December 26, 2024.
(D) A statement that the proposed contract is within the appropriated budget authority for the
agency for the fiscal year and is consistent with the financial plan and budget adopted in

The Office of the Chief Financial Officer hereby certifies that the sum of $752,133.49 is included in
the District's Local Budget and Financial Plan for Fiscal Year 2025 to fund the costs associated with
the Department on Disability Services’ human care agreement (HCA) with Wholistic Home &
Community Serivces , Inc. This certification supports the occupancy-related residential expenses
for District of Columbia persons with intellectual and developmental disabilities receiving
residential habilitation or supported living HCA during the period from March 1, 2025, through
February 28, 2026. The total fund allocation in the amount of $1,282,844.50 is as follows:

Upon a
pproval of the District’s local budget and financial plan by the Council and the Mayor,
and completion of the 30-day Congressional layover, funds will be sufficient to pay for fees and
costs associated with the HCA. There is no fiscal impact associated with the HCA. Should you

14. Description of Amendment/Modification (Organized by UCF Section headings, including solicitation/contract subject matter where feasible.)
A. The Government of the District of Columbia hereby exercises its option to extend the term of Human Care Agreement
(HCA) No. CW101973, pursuant to Section F.3, Option to Extend Term of the Agreement, for option year two (OY2)
from March 1, 2025 through February 28, 2026.
B. In accordance with section 38(d) of the Standard Contract Provisions (Way to Work Amendment Act of 2006) the
living wage rate is hereby increased to 17.50 and on July 1, 2025, the living Wage and the minimum wage rates will
increase to $18.00 per hour.
C. The not-to-exceed HCA amount of OY1 is $1,282,844.50.
D. All other terms and conditions shall remain the same.
Except as provided herein, all terms and conditions of the document is referenced in Item 9A or 10A remain unchanged and in full force and effect.
