Pursuant to section 451 of the District of Columbia Home Rule Act (D.C. Official Code § 1-
204.51) and section 202 the Procurement Practices Reform Act of 2010 (D.C. Official Code § 2-
352.02), enclosed for consideration and approval by the Council of the District of Columbia is
proposed Modification No. M00004 to Human Care Agreement No. CW104674 with Innovative
Life Solutions, Inc. to exercise option year two in the not-to-exceed amount of $1,614,292.68. The
period of performance is from April 1, 2025, through March 31, 2026.
Under the proposed modification, Innovative Life Solutions, Inc. will continue to provide
residential services not covered by Medicaid for individuals with intellectual and developmental
disabilities.

recovers any outstanding debt as provided under D.C. Official Code § 2-353.01(b):
The Citywide Clean Hands database certified contractor as being in compliance with the Office of
Tax and Revenue and the Department of Employment Services taxes on January 15, 2025.
(D) A statement that the proposed contract is within the appropriated budget authority for the
agency for the fiscal year and is consistent with the financial plan and budget adopted in

the District's Local Budget and Financial Plan for Fiscal Year 2025 to fund the costs associated with
the Department on Disability Services’ human care agreement with Innovative Life Solutions, Inc.
This certification supports the occupancy related expenses during the period from April 1, 2025,

A. The Government of the District of Columbia hereby exercises its option to extend the term of Human Care Agreement
(HCA) No. CW104674, pursuant to Section F.3, Option to Extend Term of the Agreement, for option year two (OY2)
from April 1, 2025 through March 31, 2026.
B. In accordance with section 38(d) of the Standard Contract Provisions (Way to Work Amendment Act of 2006) the
living wage rate is hereby increased to $17.50 effective January 1, 2025 through June 30, 2025. On July 1, 2025, the

C. The not-to-exceed HCA amount of OY2 is $1,614,292.68.
D. All other terms and conditions shall remain the same.
