Dear Chairman Mendelson:
Pursuant to section 451 of the District of Columbia Home Rule Act (D.C. Official Code § 1-
204.51) and section 202 of the Procurement Practices Reform Act of2010 (D.C. Official Code§
2-352.02), enclosed for consideration and approval by the Council of the District of Columbia is
proposed Modification No. M0004 to Human Care Agreement No. CWl 08863 with La Clinica
del Pueblo, Inc., to exercise option year two in the not-to-exceed amount of $1,600,000. The period
of performance is from July 6, 2025, through July 5, 2026.
Under the proposed modification, La Clinica del Pueblo, Inc., will continue to provide substance
use disorder level 1-2.5 outpatient treatment services to District consumers.

The purpose of this Human Care Agreement (HCA) Modification for the above-referenced HCA is as follows:
A. In accordance with 27 DCMR § 2008 and Section F. 3, “Option to Extend the Term of the Agreement,”
Option Year Two is hereby exercised for the period of performance from July 6, 2025, through July 5,
2026, in the not-to-exceed amount of $1,600,000.
B. Funding of the HCA beyond September 30, 2025, is contingent upon the availability of funds in the DBH
Fiscal Year 2026 budget that will be confirmed by issuing a Purchase Order.
Except as provided herein, all terms and conditions of the document is referenced in Item 9A or 10A remain unchanged and in full force and effect.
