Dear Chairman Mendelson:
Pursuant to section 451 of the District of Columbia Home Rule Act (D.C. Official Code § 1 -
204.51) and section 202 of the Procurement Practices Reform Act of 2010 (D.C. Official Code §
2-352.02), enclosed for consideration and approval by the Council of the District of Columbia is
proposed Modification No. M06 to Contract No. CW92321 with Qlarant Quality Solutions, Inc.
to exercise option year four in the amount of $1,754,462.97. The period of performance is from
December 1, 2025, through November 30, 2026.
Under the proposed modification, Qlarant Quality Solutions, Inc. will continue to conduct provider
certification reviews for the U.S. Department of Health & Human Services, Centers for Medicare
and Medicaid Services Home and Community Based Services Waiver residential program
providers.

A. The Government of the District of Columbia hereby exercises its option to extend the term of Contract No. CW92321,
pursuant to Section F.2, Option to Extend Term of the Contract, for option year four (OY4) from December 1, 2025
through November 30, 2026.
B. The not-to-exceed contract amount of OY4 is $1,754,462.97.
C. Replace the U.S. Department of Labor Wage Determination No. 2015-4281, Revision No.29, dated 04/11/2024. with
the U.S. Department of Labor Wage Determination No. 2015-4281, Revision No.34, dated 07/08/2025.
D. Replace The Living Wage Act Notice effective July 1, 2024, with The Living Wage Act Notice effective July 1, 2025.
On July 1, 2025, the Living Wage and the Minimum Wage Rates increased to $17.95 per hour, found at the following
