Pursuant to section 451 of the District of Columbia Home Rule Act (D.C. Official Code § 1 -
204.51) and section 202 of the Procurement Practices Reform Act of 2010 (D.C. Official Code §
2-352.02), enclosed for consideration and approval by the Council of the District of Columbia is
proposed Modification No. M0004 to Human Care Agreement No. CW104624 with MBI Health
Services, LLC, to exercise option year three in the not -to-exceed amount of $3,500,000. The
performance period is from December 28, 2025, through December 27, 2026.
Under the proposed modification, MBI Health Services, LLC will continue to provide mental
health rehabilitation services.

SUBJECT: Certification of Funding availability for MBI Health Services, LLC $3,500,000
The Office of the Chief Financial Officer hereby certifies that the sum of $3,500,000 is included in the District’s
Local Budget and Financial Plan submitted to Congress by the Chairman of the Council to the Speaker of the
House of Representatives for Fiscal Year 2026 to fund the costs associated with the Department of Behavioral
Health Human Care Agreement with MBI Health Services, LLC. This certification supports the Mental Health
Rehabilitation Services Human Care Agreement during the period from December 28, 2025, through December
27, 2026. The fund allocation is as follows:

Upon approval of the District’s Local Budget and Financial Plan by the Council and the Mayor and
completion of the thirty-day Congressional layover, funds will be sufficient to pay for fees and costs
associated with the contract. There is no fiscal impact associated with the contract.

The Government of the District of Columbia hereby modifies Human Care Agreement No. CW104624 as follows:
A. In accordance with 27 DCMR § 2008 and Section F.3 Option to Extend the Term of the Agreement, the District
hereby exercises Option Year Three (OY3) for the period of performance from December 28, 202 5, through
December 27, 2026, in the not-to-exceed amount of $3,500,000.
B. Continuation of OY3 services beyond FY26 period of performance, from October 1, 2026, through December
27, 2026, is pending appropriations/availability of funds under the DBH FY2 7 budget which will be reflected
in a District issued Purchase Order.
Except as provided herein, all terms and conditions of the document is referenced in Item 9A or 10A remain unchanged and in f ull force and effect.
