included in the District's Local Budget and Financial Plan for Fiscal Year 2026 to fund the costs
associated with the Department on Disability Services’ contract with Ward & Ward Mental
Health Services, LLC. This certification supports the occupancy-related residential expenses for
District of Columbia persons with intellectual and developmental disabilities receiving
residential habilitation or supported living contract during the period from June 1, 2026, through
May 31, 2027. The total fund allocation for option year one in the amount of $1,194,367.45 is as

14. Description of Amendment/Modification (Organized by UCF Section headings, including solicitation/contract subject matter where feasible.)
A. The Government of the District of Columbia hereby exercises its option to extend the term of Human Care
Agreement (HCA) No. CW123511 pursuant to Section F.2, Option to Extend Term of the Agreement, for
option year one (OY1) June 1, 2026 through May 31, 2027.
B. The not-to-exceed HCA amount of OY1 is $1,194,367.45.
C. On July 1, 2025, the Living Wage and the Minimum Wage Rates increased to $17.95 per hour. The revised
Living Wage Act Notice and Fact Sheet effective July 1, 2025, have been posted on the OCP intranet under
“Required Solicitation Documents.”
D. Replace the U.S. Department of Labor Wage Determination No. 2015-4281, Revision No.26, dated
05/10/2023, hereby incorporated into this contract, with the U.S. Department of Labor Wage Determination
No. 2015-4281, Revision No.35, dated 12/03/2025.
E. All other terms and conditions remain the same.
Except as provided herein, all terms and conditions of the document is referenced in Item 9A or 10A remain unchanged and in full force and effect.
