Pursuant to section 451 of the District of Columbia Home Rule Act (D.C. Official Code § 1-
204.51) and section 202 of the Procurement Practices Reform Act of 2010 (D.C. Official Code §
2-352.02), enclosed for consideration and approval by the Council of the District of Columbia is
proposed Modification No. M0003 to Human Care Agreement No. CW121 802 with PurFoods,
LLC dba Mom’s Meals to exercise option year one in the not-to-exceed amount of $8,106,577.40.
The period of performance is from June 23, 2026, through June 22, 2027.

Under the proposed modification, PurFoods, LLC dba Mom’s Meals will continue to prepare and
deliver fresh, nutritious, and ready-to-eat refrigerated home-delivered meals to seniors aged 60 and
older.

This modification to Human Care Agreement CW121802 identified in Block 10A above, is hereby modified as
follows:

1.In accordance with Human Care Agreement No. CW121802, Section D.3, Option to Extend the Term of the
Agreement, the Government of the District of Columbia hereby exercises option period one to extend the term
of the Human Care Agreement from June 23, 2026, through June 22, 2027, in the not-to-exceed amount of
$8,106,577.40.

2. Section H, Attachment H.2 is revised to incorporate the U.S. Department of Labor Wage Determination,
Labor Wage Determination No. 2015-4281, Revision No. 35, dated December 3, 2025, available at
https:// sam.gov/wage-determination/2015-4281/35 .

3. In accordance with section 38(d) of the Standard Contract Provisions (Way to Work Amendment Act of
2006), the Living Wage is hereby adjusted to $17.95 per hour effective January 1, 2026, until June 30, 2026.
Effective July 1, 2026, the District Minimum Wage and Living Wage will increase to $18.40 per hour.
