(identity theft statute and security breach notification statute) to
include the same data elements
in each
as follows:

Tribal identification card;
Federal or state government issued identification card;
Shared secrets or security tokens that are known to be used for data based authentication;

mail address, in combination with a password or security question and answer that would permit access to an online account;
A birth or marriage certificate;
Medical information

Unique biometric data

An individual taxpayer identification number.

Unauthorized use of personal identifying information; penalties; restitution.

(b)

As used in this section "personal identifying information

" means the name
, address, telephone number, driver's license number, social security number, place of employment, employee identification number, tribal identification card number, mother's maiden name, demand deposit account number, savings account number, or credit card number

or any of the following
data elements
of an individual person

(iv)

Driver's license number

(v)

Account number, credit card number or debit card number in combination with any security code, access code or password that would allow access to a financial account of the person;
