Issuance of license; fee; duration; renewal; transferability; provisional licenses; procedures.

(a)

The division shall issue a license under this act:
(ii)

Upon payment of a license fee as established by the department for each health care facility. The department shall adopt rules which provide for reasonable fees not to exceed
five hundred dollars ($500.00)
five thousand dollars ($5,000.00) for initial licensure and changes to licenses and one thousand dollars ($1,000.00) for renewal
